Student Health Records:

Liberty University cares about the well-being of its students. To maintain the health of our community, Liberty requires students to be immunized according to the Centers for Disease Control and Prevention (CDC) guidelines. Immunizations and medical history must be documented through your Student Health Form. Liberty University provided Medical and Religious Exemptions are accepted and can be found on the Student Health Form.

Accessing the Student Health Form

1. Log into your ASIST account.
2. Click “Student”.
3. Select “Student Health Form”.
4. Complete the Immunizations, Medical History, & TB Questions sections.

Please review all the instructions for the Student Health Form before completing it.

Please note that our office must be able to authenticate the origin of your immunization documentation in order for your records to be approved. Documentation should be uploaded as a PDF or JPEG file.

Fee Notice: Students who do not complete all three sections will have a $50 health records late fine added to their student account.
